Spectrum Health Care’s stroke care services are designed to provide support and clinical care that fosters independence and positive recovery. Our stroke care services are available 24 hours a day, 7 days a week and are provided by trained nurses, physiotherapists and Personal Support Workers who understand the commitment and dedication required when caring for patients/clients who have survived a stroke.
Are stroke care services right for you/your loved one?
If you/your loved one have been discharged from a hospital or rehabilitation facility and require stroke aftercare services, our team of dedicated health care professionals can develop a customized care plan that is flexible and adaptable to the various stages of your/their recovery. Our care plans are tailored to the needs of each client and include the following services:
- Nursing: To aid patients/clients in reaching their maximum recovery potential through continuous monitoring, assessment of care plan, pain and symptom management and education
- Physiotherapy Services: To help patients/clients regain mobility and ranges of movement
- Personal/Home Support: To assist with activities of daily living including: meal preparation, housekeeping, personal hygiene, toileting and errand running
- Family Caregiver Relief: To provide continued care while a primary caregiver runs an errand or takes a break
