Advanced Illness and Palliative Care

The diagnosis of a palliative or advanced illness can often be challenging and complex. Spectrum Health Care’s advanced illness and palliative care services are designed to support the unique needs of a patient's diagnosis by focusing on the physical, emotional and spiritual care required for a patient and their family support system.

Our advanced illness and cancer palliative care services are available 24 hours a day, 7 days a week and are provided by a multi-disciplinary team of health professionals.

Our advanced illness, palliative care and palliative nursing services include:

  • 24/7 access to our interdisciplinary team of clinicians, including physicians, nurse practitioners, nurses, personal support workers, physiotherapists, companions, paramedics, wellness practitioners and spiritual/bereavement care providers
  • Dedicated program managers to provide ongoing assessments of care plans
  • Family caregiver relief and practical supports
  • Information services to help your loved one navigate the health care system
