Navigating Health Care

Navigating the health care system can be a challenge but we are here to assist you with learning about what services are right for your loved one, what resources are available and how you can learn more.

We developed Total Care to support our patients through all stages of life. Total Care also means we are committed to help your loved one find the right services.

We will help navigate the options in health care with the support of our coordinators and supervisors, who will connect with you on first visit to share with you our patient handbook, learn more about your loved one's medical history and assist your loved one with providing the right care at the right time.
