Spectrum Health Care’s advanced illness and palliative care services are designed to support the unique needs of a patient's/client’s diagnosis by focusing on the physical, emotional and spiritual care required for a patient/client and their family support system. Our advanced illness and cancer palliative care services are available 24 hours a day, 7 days a week and are provided by a multi-disciplinary team of health professionals.

- 24/7 access to our interdisciplinary team of clinicians, including: physicians, Nurse Practitioners, nurses, Personal Support Workers, physiotherapists, companions, paramedics, wellness practitioners and spiritual/bereavement care providers
- Dedicated program managers to provide ongoing assessments of care plans
- Family caregiver relief and practical supports
- Information services to help you/your loved one navigate the health care system
